Hi and Welcome! I've enjoyed your paintings that you've posted so far - they have a certain liveliness to them. Are you still working on either of these (this one or the Doctor)?
What strikes me on this piece is the brightness of the whites of the eyes and the teeth. The teeth especially seem to jump out of his mouth at the corners. Giving then some graduated shading as they round would help to push them back into the mouth and to conform with the curve of his face. The whites of the eyes could use some shading as well.
Are you working from a reference photo that was taken with a flash? The flash will wash out a lot of the shadows that indicate the form. If you look in the photography section there's a wealth of information on how to take and what makes a good reference photo. I've found it incredibly informative!
